I, the Team Contact as listed on the online Registered Roster with PlaySloPitch (the governing body), hereby agree to provide a copy of this Release and Indemnity as set out below to the players listed on my Registered Roster for their review.
I, as a player, manager or coach listed on a PlaySloPitch Registered Roster, on behalf of myself, members of my family, my heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless Guelph SloPitch, PlaySloPitch, Slo-Pitch Ontario Association, Slo Pitch National, GOOD TO GO SLO PITCH o/a G.T.C.S.P.L. and any of its representatives and/or agents for any injury, loss or damage to my person or property howsoever caused, arising out of or in connection with my taking part in slo-pitch activities and notwithstanding that the same may have been contributed to or occasioned by the negligence of PlaySloPitch/Good to Go Slo Pitch/GTCSPL/Guelph SloPitch, its representatives or agents. I understand that PlaySloPitch, Guelph SloPitch and/or GTCSPL reserve the right to publish the name and photographs of any participants involved in PlaySloPitch events. I am aware that failure to abide by the rules and the Code of Conduct may result in disciplinary action being taken against me in the way of fine(s) or suspension(s) from the programs offered by PlaySloPitch and such suspension(s) may be for any duration decided by the PlaySloPitch Disciplinary Committee. In addition, pursuant to the Information and Protection of Privacy Act, I consent to the use by PlaySloPitch of the information about me for the purpose of slo-pitch related activities.
By signing this form, I agree to abide by all rules and regulations as set forth by The City of Guelph, Township of Guelph Eramosa, Guelph SloPitch, PlaySloPitch, Good To Go Slo Pitch (o/a G.T.C.S.P.L.) and all representatives of same. I shall inform my team of all rules and by my signing; they too shall be in agreement. G.T.C.S.P.L. & all Executive Members and Affiliated representatives & The City of Guelph & Township of Guelph Eramosa shall NOT be held responsible or liable for any injuries to any player, spectator or otherwise and shall not be responsible for any articles lost or stolen.
Furthermore, it is understood that before, after and during any game play, be it regular season or tournament, the aforementioned parties are not liable for any fines, injuries or otherwise due to alcohol related responsibility of individual players, teams or spectators with the exception of inside the designated licensed area of a sanctioned tournament with an approved liquor license.
